Mastering VPT: Step-by-Step Techniques for Pulp Preservation
Dr. Domenico Ricucci
This demonstration masterclass will focus on the essential clinical steps of Vital Pulp Therapy (VPT). Participants will learn how to perform direct pulp capping, partial pulpotomy, and full pulpotomy using specially designed tooth models. Emphasis will be placed on critical success factors, including complete caries removal and strict aseptic technique. Materials and methods that streamline the procedure and enhance clinical outcomes will be introduced and demonstrated. A step-by-step workflow will be illustrated, with particular attention to the proper placement of bioceramic materials on the pulp wound and the subsequent base layers. Attendees will gain a clear understanding of how to choose the most appropriate VPT approach for different clinical scenarios. The course will also provide guidance on patient communication and management, as well as considerations for prognosis.

Restorative Decision-Making and Execution for the Endodontically Treated Tooth
Dr. Alan Atlas
This demonstration masterclass focuses on the clinical execution of restorative strategies following endodontic treatment, with emphasis on decision-making grounded in structural foundation, functional load, and adhesion.
Participants will observe complete direct and indirect restorative workflows, examining how preparation design, material selection, and the timing of foundational build-ups influence structural integrity and long-term performance. Particular attention is given to the impact of endodontic procedures on the bonding substrate and how this affects restorative predictability.
Live, step-by-step demonstrations will illustrate posterior composite techniques, core development, and post placement when indicated, with a focus on simplifying techniquesensitive steps and improving consistency in clinical outcomes. The objective is not to introduce new complexity, but to refine clinical workflows through a deliberate, evidence-based approach. From decision-making to execution, this masterclass provides practical, transferable workflows that can be implemented immediately to improve restorative predictability.

Updates of minimally invasive and predictable removal of separated instruments
Dr. Yoshi Terauchi
Instrument retrieval may be unpredictable unless the standardized protocol is followed. The recent study on instrument retrieval revealed that removal of visible broken instruments is 100% predictable following the standardized protocol. It was also shown from the study that the longer the broken instrument, the longer time it took to remove it. Therefore, removal of longer broken instruments requires different techniques from that of shorter ones. It is also essential to preserve as much tooth structure as possible to prevent root fracture and perforation. In the workshop unique techniques for instrument retrieval using “TFRK” or martensitic NiTi rotary files will be presented to make the instrument retrieval highly predictable and minimize dentin sacrifice. The instrument retrieval techniques presented in the workshop are also supported by evidence because of predictability and higher success rates compared to the traditional techniques.

Intentional Replantation: Contemporary Techniques and Clinical Decision-Making
Dr. Samuel Kratchman
What was once considered a procedure of last resort, Intentional Replantation can now be included into one’s everyday treatment planning. With the reality of potential issues with implants like peri-implantitis, esthetic concerns in anterior region, etc., it is more important than ever to try and save one’s natural teeth. While describing the new techniques versus the way Replantations used to be performed, the goal of the lecture is to become confident doing this procedure. The importance of proper case selection will be discussed along with a step-by-step technique, offering tips to success, and showing recommended armamentarium. There will be many clinical cases shown demonstrating various indications, proving this to be a predictable procedure, supported by decades of literature. Saving our patient’s natural teeth should still be the goal of any dentist/endodontist.

Clinical Decision-Making in Refractory Cases: Retreatment or Surgery
Dr. Meetu Kohli
The objective of this presentation is to evaluate the options that the spectrum of endodontic retreatment offers for preserving natural dentition. Modern clinical practice requires dentists to be familiar with advances in materials, instruments, and procedures in order to help their patients choose the most beneficial treatment option in the long term, in terms of maintaining health and function.
Thanks to advances in dental materials and surgical techniques, endodontic microsurgery has achieved more predictable outcomes. However, there are specific cases in which surgical endodontics does not replace conventional retreatment. Successful treatment requires a thorough understanding of the causes of failure, the prognosis of each approach, and the technologies and materials that enable the best possible result.
This presentation will review the relevant literature evaluating the prognosis of both microsurgical and conventional retreatment, helping to ensure that the decision-making process is well informed.

Management of challenging non-surgical retreatment cases
Dr. Yoshi Terauchi
Initial root canal therapy could go wrong non-healing for a multitude of reasons. Retreatment could also result in another failure unless the causes of the failure are identified and eliminated. Some of the non-surgical retreatment cases could result in persistent periapical periodontitis due to the bacteria surviving the endodontic treatment in the difficult-to-reach spaces that are often apical to the broken instrument or the ledge formation, or the bacteria forming biofilms on the root filling materials and necrotic pulp tissues, which in the end expands beyond the apical foramen. In fact, many studies regarding persistent apical periodontitis showed the presence of intracanal biofilms in the majority of cases with large periapical lesions. The key to success in non-surgical retreatment is hence to remove or reduce the bacteria harboring in those spaces. The biggest challenge in retreating cases with persistent apical periodontitis is to debride those spaces inaccessible to any instruments available on the market. However, if you could manage the impediments that are not visualized under the microscope, the access to those difficult-to-reach spaces could be made possible, followed by elimination of the bacteria casing persistent apical periodontitis. Therefore, in the lecture, the ideal techniques to manage those challenging non-surgical retreatment cases will be discussed and presented.

Bridging the Gap: Evidence-Based Restorative Strategies to Strengthen the Restorative–Endodontic Connection
Dr. Alan Atlas
Inadequate coronal restoration remains one of the most common causes of failure in endodontically treated teeth. This program challenges traditional restorative concepts and presents a biologically and mechanically driven, evidence-based approach to improving longterm outcomes.
Participants will learn how to make more predictable restorative decisions by focusing on threecritical variables: remaining tooth structure, functional load, and adhesion timing. Emphasis is placed on optimizing the bonding substrate—often overlooked in conventional workflows—and understanding how endodontic procedures can influence restorative success.
The course provides practical, step-by-step protocols for Class I and II composite restorations placed either before or after obturation, highlighting the advantages of pre-endodontic build-up strategies. For indirect restorations, participants will learn simplified, research-driven approaches to post selection, post space preparation, and core development using a novel fiber post system designed to reduce technique sensitivity and improve efficiency.

Vital Pulp Therapy in Deep Caries: From Histopathology to Clinical Decision-Making.
Dr. Domenico Ricucci
When managing deep caries lesions approaching the pulp chamber, clinicians are often faced with the dilemma of whether to preserve or remove the pulp tissue. Traditionally, teeth diagnosed with irreversible pulpitis have been managed with root canal treatment. However, emerging evidence challenges this paradigm and opens new possibilities for vital pulp therapy, even in cases previously considered beyond conservative management.
This lecture explores clinical strategies for preserving pulp vitality in teeth diagnosed with either reversible or irreversible pulpitis. Drawing from extensive histopathological and histobacteriological research, a novel approach is proposed, based on intraoperative decision-making guided by magnified visual assessment of the exposed pulp. This technique allows clinicians to estimate the extent and location of the infection front and selectively remove only the diseased tissue, preserving the healthy pulp. The exposed wound is then sealed with biocompatible, potentially bioactive materials, promoting healing and long-term pulp preservation.
