The next global meeting Endo Sapiens by Domenico Ricucci will be held on October 3-5, 2025 in Amsterdam, Netherlands. The meeting will take place in the largest congress center of Amsterdam - RAI. The main scientific program will be held on October 4th and 5th. The pre-congress demo masterclasses will be held on October 3rd.

Amsterdam is synonymous with vibrant culture, rich history, inclusiveness and beautiful canals. Since its construction in the 17th century, the Amsterdam canal ring has grown into one of the most unique urban landscapes in the world. It’s not for nothing that it has been designated a UNESCO World Heritage Site.

The next Endo Sapiens forum is moving from Paris to Amsterdam in 2025. We look very much forward to seeing you there on October 3-5, 2025.

The recent edition of Endo Sapiens 2024 in Paris was attended by more than 500 delegates from 61 countries, literally from all over the world! It is with immense pride that I announce the 4th International endodontic forum Endo Sapiens 2025 which will be held on October 3 to 5, 2025 in Amsterdam, Netherlands. It is an honor for me to introduce such an outstanding scientific speakers team: José Siqueira, Franklyn Tay, Mitsuhiro Tsukiboshi, Meetu Kohli, Jorge Vera, Yoshi Terauchi, Roeland De Moor and myself. Endo Sapiens 2025 will take place in the the largest and most famous congress venues of the city - RAI Amsterdam.

RAI Amsterdam has a rich history. What started in 1893 as an association for the bicycle industry has grown into an internationally leading trade fair and conference organisation in just over a century. Many valuable meetings have taken place, often with prominent people. For example, at a public meeting in 1968, Martin Luther King addressed hundreds of people about change. During AIDS2018, Bill Clinton and Prince Harry, among others, spoke to those present about the impact of the global epidemic. Major bands and artists have taken its stage, such as Frank Sinatra, Ike & Tina Turner and U2. During these events, people have been inspired and/or knowledge was shared. Over all these years, RAI has gained enormous experience in how to set this in motion.

Domenico Ricucci & Meetu Kohli pre-congress Demo masterclass
Execution of Non-Surgical Retreatment: step by Step demonstration from perspective of two expert clinicians.

The aim of this demonstration is to review critical procedural steps in endodontic retreatment. Non-surgical retreatment under microscope from the pre-endodontic build up and isolation to post endodontic restoration will be demonstrated. Materials and techniques will be introduced that help the clinician conduct the treatment in a streamline fashion optimizing success from restorative and root canal treatment point of view. The key to successful retreatment is adequate disinfection. To allow for irrigation and medication to be effect against the persistent intracanal infection, the previously filled gutta percha and sealer have to be efficiently removed. Ensuring correct working length and adequate disinfection of the apical third of the root canal is critical to a successful outcome.

Yoshi Terauchi pre-congress Demo masterclass
Instrument retrieval may be unpredictable unless a standardized protocol is followed. Removal of visible broken instruments may be 100% predictable following precise guidelines. In addition, longer separated instruments present more difficulties and require different techniques. Any technique should aim at preserving as much tooth structure as possible to prevent root fracture and perforation. In this demonstration course, unique techniques for instrument retrieval will be presented to make the retrieval highly predictable and minimize dentin sacrifice. Participants will learn how to make an accurate evaluation of each case with instrument separation, establish a precise treatment plan, and adopt the most adequate techniques to successfully remove the separated instrument with the minimum sacrifice of sound tooth structure.

    Cetraro, Italy
    Since 1998 Dr. Ricucci has run his own histology laboratory and his primary research interest relates to pulpal and periapical tissue reactions to caries and treatment procedures, biofilms in endodontic infections, etiology of RTC treatment failure, pulp regeneration / revascularization. Dr. Ricucci has published 112 papers and has lectured both nationally and internationally.
  • José F. Siqueira Jr.
    Rio de Janeiro, Brazil
    Dr. Siqueira is the chair of endodontics, director of the postgraduate program in endodontics, and head of the molecular microbiology laboratory at Iguaçu University, in Rio de Janeiro, Brazil. He is also professor of the postgraduate program in endodontics at Grande Rio University, also in Rio de Janeiro. Dr. Siqueira has authored several books on endodontics and microbiology, several chapters for leading international endodontic textbooks and more than 430 scientific papers. His primary research interests are in the areas of endodontic microbiology and infection control.
  • Franklin Tay

    Dr. Franklin Tay is an academic clinician and researcher in the field of dental medicine who currently holds the titles of Professor and Chairman in the Department of Endodontics at the Dental College of Georgia – Augusta University. He also serves as a professor in the Department of Oral Biology and the College of Graduate Studies at the same institution.
  • Mitsuhiro Tsukiboshi
    Nagoya, Japan
    Dr. Tsukiboshi is a graduate of Osaka University School of Dentistry in 1976. He earned a PhD in dental materials from Kyoto University in 1980 and has been engaged in private practice since 1981. Dr. Tsukiboshi's interest in dental traumatology and autotransplantation has led to extensive clinical involvement, course presentations and publications. Dr. Tsukiboshi is certified as an adjunctive assistant professor of Western university in USA in 2012 and a clinical professor of the dental school of Tohoku University from 2015-2018.
  • Yoshi Terauchi
    Tokyo, Japan
    Dr. Terauchi is a part-time Professor at Tokyo Medical & Dental University and maintains a private practice limited to endodontics since 1998 in Tokyo, Japan. He earned his DDS in 1993 and completed his residency at Tokyo Medical & Dental University in 1995, where he also received his PhD from the Department of Endodontics. He has published numerous articles in peer- reviewed journals nationally and internationally. He also authored in several chapters in textbooks including the 11th edition of Pathways of the Pulp.
  • Jorge Vera

    Clinical Assistant Professor and Director of the Advanced Education Program in Endodontics, Department of Periodontics and Endodontics, University at Buffalo School of Dental Medicine. Educator member of the American Association of Endodontists. Postgraduate Endodontic Certificate from Tufts University, School of Dental Medicine, Boston Mass. USA 1993, Recipient of the Samuel Seltzer award from the Oral Surgery, Oral Medicine, Oral Pathology, Oral Radiology and Endodontics Journal in 2005.     Dr. Meetu Ralli Kohli is Clinical Associate Professor and Director for Continuing Education and International Visiting Scholar Program at the University of Pennsylvania, School of Dental Medicine, Endodontic department. She is a member of the Dean’s advisory council, School of Dental Medicine, UPENN. She is on the Scientific Advisory Board as a reviewer for Journal of Endodontics and International Endodontic Journal. Dr. Kohli has published in national and international peer reviewed journals and has contributed in books on microsurgical retreatment.
  • Roeland De Moor
    Ghent, Belgium
    Dr. De Moor is double qualified with a specialization certificate in endodontics (UGent) & surgical endodontics (ACTA / Academic Center Amsterdam) and a specialization training in Pediatric Dentistry - Traumatology (UGent). His teaching assignment at Ghent University includes Endodontics, Restorative Dentistry, Dento-Alveolar Traumatology and Liability, where he is a full professor.
  • Guillaume Jouanny
    Paris, France
    Dr. Jouanny is an endodontist with extensive academic and professional experience. He earned two certificates in Conservative Dentistry and Endodontics, as well as in Prosthetic Dentistry from Université Paris René Descartes. In 2015 he completed a Postgraduate Program in Endodontics at the University of Pennsylvania in Philadelphia. He teaches at the university Paris René Descartes. Since 2015, he has been running an exclusive endodontic practice in Paris, France. Guillaume Jouanny is an internationally recognized speaker, having presented at conferences in over 30 countries.
  • Bertrand Khayat
    Paris, France
    Dr. Khayat received his DDS degree from the University of Paris, France. He then obtained his Certificate in Endodontics and his Master of Science in Dentistry from the University of Washington. He has published numerus articles in national and international journals. He has lectured extensively nationally and internationally in 50 countries.
    He is currently Adjunct Assistant Professor at the University of Pennsylvania, Department of Endodontics while maintaining a private practice in Conventional and Surgical Endodontics in Paris.

Eradicating or controlling endodontic infection: What is the attainable goal

Apical periodontitis is primarily caused by bacteria infection of the necrotic root canal system. In teeth with vital pulps there is no infection in the root canal space, thus the therapeutical approach in these cases is completely different from infected teeth. Infection is often established as a biofilm covering the walls of the main canal lumen, and can spread to other distant areas of the canal system that are difficult to reach and may require especial treatment strategies. If bacteria persist in the root canal after treatment, there is an increased risk of failure. This presentation will cover the most relevant aspects of root canal infection, including progression of necrosis/infection in the root canal space, and the diversity, location, morphology and behavior of bacterial biofilms. The limitations in eradicating infection will also be revealed, including a discussion on the implication of residual infection. What happens when bacterial load has been reduced but infection has not been completely eradicated? Does healing occur even with a reduced amount of bacteria surviving in the root canal system?

Value-added root canal disinfection with lasers: present status and future perspectives

The impact of light used for bacterial biofilm eradication is know for long. Photodynamic therapy or Photo-activated disinfection (PAD) was already initiated in the beginning of the previous century. Around the 1980’s the use of dental lasers for biofilm interactions gained renewed attention. The introduction of laser cavitation induced fluid streaming (laser activated irrigation - LAI) rang in a new era at the beginning of this century.
Laser light is unilateral, meaning there is no side firing. As we rely on fibers with straight forward emission, solutions for better 3D laser-biofilm interaction are needed. Both LAI (even with the fiber outside the root canal lumen i.e. PIPS or SWEEPS) and PAD (impregnation of the biofilm with a dye before exposure to laser light) pick up on these shortcomings. Light activated nanoparticles are now investigated to overcome the shortcomings of PAD.

Is removal of separated instruments always necessary?

Instrument retrieval may sometimes be frustrating because it often implies the sacrifice of consistent amounts of sound dentin, which may lead to iatrogenic accidents such as perforation. Within this presentation, the different microbiologic conditions of the root canal system that can be present at inception of the endodontic treatment are illustrated. It is stressed how vital, although inflamed, pulp tissue does not harbor bacteria, for which reason a sterile instrument separated in a canal with previously vital pulp is unlikely to cause treatment failure. Under such circumstances, depending on the location of the separated segment, attempts to remove it may be more risky than leaving it and a “wait and see” strategy is indicated. On the contrary, when separation occurs in a canal with necrotic/infected pulp and apical radiolucency, infection control in the whole canal length is prevented. Therefore, attempts to remove the fractured instrument must be undertaken.

This lecture will examine the etiology and mechanisms by which coronal leakage compromises root canal therapy, shedding light on its role in bacterial reinfection and the breakdown of obturation materials. Strategies for prevention, including advancements in restorative techniques, material selection, and clinical protocols, will be emphasized. Attendees will gain insights into evidence-based approaches to mitigate coronal leakage, ensuring the long-term success of endodontic treatments.

Surgery or Not: Making the decision for optimal success

The aim of this presentation is to evaluate options that the endodontic retreatment spectrum provides to retain natural dentition. Modern clinical practice requires dentists to be familiar with new developments in materials, instruments and procedures to assist their patient in electing the most beneficial long-term treatment option in terms of maintaing health and function. Endodontic Surgery has evolved tremendously in the last decade, into a microsurgical procedure. Due to advancements in dental materials and surgical techniques, surgical micro-endodontics has greater predictable outcomes. However, there are specific cases, for which surgical endodontics is not a replacement for conventional retreatment. Successful treatment requires a thorough understanding of the causes of failure, the prognosis of each approach, technologies and materials that allow for the best possible outcome. This presentation will review the relevant literature, which evaluates prognosis of microsurgical and conventional retreatment which will help the decision-making process an informed one.

Efficacy of intentional replantation and autotransplantation for endodontically compromised teeth.

Clinicians may sometimes face severely compromised teeth in which orthograde endodontic treatment or apical microsurgery are not feasible. In these situations the choice of intentional replantation (IR) can be considered, if the involved tooth has simple root form for smooth extraction. In some instances, when a donor tooth is found in the same mouth, autotransplantation (ATT) could be the more optimal option.
In this presentation, the indications, advantages of IR and ATT will be extensively discussed. The treatment of clinical cases with varying severity will be thoroughly illustrated.

Controversies in healing and lack of healing of the periradicular tissues.

A common clinical dilemma in endodontics arises when a small, asymptomatic periapical radiolucency is observed, often visible as a hypodense area on CBCT images, despite the case being clinically asymptomatic for a certain period. The decision of whether to treat or not depends on various factors, including understanding the etiology of the lesion, potential reasons for incomplete healing, and patient expectations. This presentation will review relevant literature on the topic and explore clinical cases to illustrate decision-making in such scenarios.

Addressing contemporary challenges in endodontic failure: Insights and concluding perspectives

When a root-treated tooth with apical periodontitis shows no symptoms but fails to exhibit radiographic healing during follow-up, clinicians face uncertainty regarding the necessity of further intervention. This presentation will illustrate diverse clinical cases examined through histopathology, shedding light on the so-called non-microbial causes of failure such as "true cysts" and "foreign body reactions." The contentious topic of healing via "scar tissue" will also be critically analyzed. Concluding with an exploration of factors contributing to the loss of endodontically treated teeth, this discussion offers evidence-based insights into complex diagnostic and therapeutic dilemmas in contemporary endodontics.
